Job DescriptionJob DescriptionDescription: Klarus Home Health & Hospice/Jet Health Inc is looking for a PRN Licensed Vocational Nurse (LPN / LVN) to join our dedicated team to provide community-based care for our patients currently receiving home health services in the San Antonio, TX region and surrounding areas. This is a weekend position, Sat/Sun.The primary function of the LVN is to provide nursing care, under the supervision of a registered nurse, for patients of all ages in their place of residence. The LVN will coordinate care with the interdisciplinary group, patient/family, and referring agency to ensure the best patient outcomes.Requirements: Key Responsibilities:Provide nursing care to patients of all ages in their homes under the supervision of a registered nurse.Coordinate care with the interdisciplinary group, patient/family, and referring agency.Assist in the development and implementation of patient care plans.Monitor patient health and report any changes to the supervising RN.Administer medications and treatments as prescribed.Educate patients and their families on health care needs, conditions, and options.Maintain accurate and timely documentation of patient care and progress.Job Qualifications:Education: Graduate of an accredited School of Vocational/Practical Nursing.Licensure: Current State license as a Licensed Vocational/Practical Nurse, or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) if practicing in a state member of the NLC. Must have a current driver's license.Experience: Two years’ experience as a Licensed Vocational/Practical Nurse in a clinical care setting. Home health experience highly preferred.EMR system: HCHB experience highly preferredSkills: Proficiency in nursing skills as defined by generally accepted standards of practice.
